We are seeking a part-time Occupational Therapist with flexible, variable availability to support our growing home health caseload in the Twin Cities and surrounding areas . Visit volume may fluctuate based on referral flow and territory , and because we are actively building our occupational therapy program, the schedule may start out slow but will steadily grow into a consistent part-time workload. Ideally, we’re looking for 2–3 days of availability each week to meet with clients and help expand OT services across our service area.

Key Responsibilities: 

• Assessment, planning, implementation, and evaluation of all occupational therapy modalities to patients.
• All duties and standards will be performed according to established policies, procedures, and competency guidelines within the department and organization.
• Improves or minimizes residual physical disabilities of the patient.
• Returns the individual to optimum and productive level within patient’s capabilities.
• Participates with all other home care personnel in patient care planning.
• Takes initial history and makes initial evaluation.
• Consults with physicians regarding patient’s progress
• Participates in quality assessment and performance improvement activities as assigned.
• Prepares clinical and progress notes.
• Helps develop the plan of care and revise, as necessary.
• Consults with family and agency personnel.
• Ability to do extensive bending, lifting and standing on a regular basis.

Requirements:

• A graduate of a OT Program recognized by the American Occupational Therapy Association or other National accepted Council or Association
• An active license to practice Occupational Therapy in MN
• A minimum of four (4) years’ experience as a Occupational Therapist, home care experience a plus
• Must have criminal background check
• Must have current CPR certification
• Must have active driver's license and clear driving record
• Computer skills.
• Ability to exercise initiative and independent judgment
