Makes follow up visits according to patient, family, and facility needs./liliCommunicates significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and/or other personnel involved with patient care./liliRevises the plan of care in response to identified patient care issues and notifies the Clinical Supervisor, physician, facility, and other team members./li/ulpWhat is Required?/pulliCurrent, unrestricted state RN licensee/lili1+ year experience in a clinical care setting as a nurse/liliAbility to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment/liliAble and willing to travel within branch/office coverage area./liliMust have a dependable vehicle, valid drivers license, and current auto insurance in accordance with state laws./liliMust be able and willing to travel 50%/liliAssociates Degree or Bachelors Degree in Nursing is preferred/liliPrevious Home Health or Hospice Experience is preferred/liliPrevious experience with HomeCare HomeBase is preferred/li/ulpYou will report to the Clinical Manager or Clinical Supervisor/ppThis is not a comprehensive list of all job responsibilities; a full job description will be provided./ppWe are committed to equal employment opportunity regardless of race, color, ancestry, religion, sex, national origin, sexual orientation, age, citizenship, marital status, disability, gender, gender identity or expression, or veteran status.
