Description

• Conduct comprehensive evaluations for patients in both inpatient and outpatient settings to design personalized treatment plans
• Implement physical therapy interventions centering on mobility, strength, balance, and functional capabilities
• Collaborate effectively with nurses, physicians, case managers, and other rehabilitation professionals to enhance outcomes
• Educate both patients and their families on therapeutic objectives, discharge planning, and at-home exercise strategies
• Maintain accurate documentation of evaluations, treatments, and patient progress in accordance with facility and regulatory standards
• Engage in care team discussions and interdisciplinary meetings to ensure seamless coordination of care

Education

• Doctorate in Physical Therapy (DPT) or its equivalent from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification preferred
• Specialized certifications (e.g., neurological, geriatric) are a bonus but not mandatory

Skills
