Nurse Care Manager

Our offices are located throughout Connecticut’s “Quiet Corner,” within easy commute of New London, Worcester, and Providence. Location: Mansfield, CT Description: United Services is looking to hire full-time Nurse Care Manager (LPN/RN). This is a great opportunity to work with individuals to meet their health and wellness goals, provide person-centered care, provide care coordination to have a positive impact on one’s life. The Nurse Care Manager will provide care management, care coordination, individual and family support, hospital transition planning and health promotion services to individuals that receive services. Schedule: Monday-Friday 8:30am-5pm with no evening, weekend or holidays required. Current Registered Nurse (RN) / Licensed Practical Nurse (LPN) license. Able to maintain and renew licensure as RN/LPN in the state of Connecticut.
