Minimum Requirements
Doctoral Degree in Physical Therapy (DPT) from an accredited program.
Active Washington State PT license is required to start the assignment.
CPR certification required.
Home health experience required. Experience conducting SOC, evaluations, ROC, recertifications, and subsequent visits.
Experience with HCHB EMR preferred.
Ability to cover patients within a 50 mile radius from base location.
Proof of eligibility to work in the U.S.

Assignment Details
Facility Type: Home Health.
Shift: Days.
Shift Hours: 08:00 - 16:00 (days).
Schedule: 5x8, 40 hours per week guaranteed.
On-call/Weekend Requirement: No.
Mileage reimbursed between patient visits from first patient to last patient.
Primary responsibilities: SOC, evaluations, return-of-care visits, recertifications, and subsequent visits.
Dress code: Business casual.
May be required to float within the local coverage area as needed.
