Care Coordinator - Children's Long-term Support Waiver Program - Ozaukee County

Job Overview Lutheran Social Services (LSS) is a private, non‑profit, human service agency serving more than 125,000 clients annually across Wisconsin and Upper Michigan. The organization provides outpatient behavioral health management, adoptions, older adult services, child, youth and family supports, community corrections programs, and services for adults with disabilities and mental health concerns. If you are motivated to serve and share your talents, this position may be for you. Position Care Coordinator (Children’s Long‑Term Support [CLTS] Waiver) – Ozaukee County, Wisconsin. The role is full‑time, Monday through Friday, first shift, with flexibility to meet family needs. Remote work is possible for up to two days a week. Essential Duties and Responsibilities Assessment of functional abilities Administer required assessment tools (e.g., functional screening)." + " Re‑assess annually or as needed. Gather information through interviews, collateral contacts, and prior records. Care / Service planning Collaborate with clients, families, supports, and service providers to develop and implement individualized service plans. Coordinate and authorize services as required. Work with team and supervisory staff to determine interventions. Communicate expectations and recommendations effectively. Care / Service facilitation Provide services in alignment with the client’s plan. Coordinate and facilitate family‑centered team meetings. Testify at legal proceedings when required. Complete documentation timely and maintain client records. Apply working knowledge of Casework principles and methods. Human behavior, group dynamics, interpersonal relations and social interactions. Social problems and their impact. Community resources. Attend and participate in Staff development programs and in‑service training. Staff and professional meetings. Clinical supervision (where required). Understand and meet productivity expectations set by the program. Perform other duties and special projects as requested. Organize and plan work effectively. Direct Service Requirements Promote client independence and growth, consistent with the service plan. Provide services using a trauma‑informed, person‑ and family‑centered model. Apply evidence‑based practices approved by agency and funder. Accurately complete time reporting. Perks Medical, dental, and vision insurance. Flexible spending for dependent & health care. Qualifications Education and Experience Bachelor’s Degree in Social Work. Bachelor’s Degree in a health and human services related field (e.g., Child Development, Community Mental Health, Counseling, Criminal Justice, Marriage and Family, Occupational Therapy, Psychology, Recreational Therapy, Rehabilitation Counseling, Social Work, Sociology, Special Education, Vocational Counseling). Minimum one year experience working with children and/or youth with disabilities or equivalent combination of academic preparation and experience. Children’s Long‑Term Support (CLTS) Requirements State certification/licensure as a social worker and one year experience with the target group. Academic degree in a health or human services field and one year experience with the target group. Minimum four years experience as a long‑term support case manager/service coordinator. Equivalent training and experience totaling four years practice or nursing degree plus one year employment with target group. Certificates, Licenses, Registrations Valid driver’s license and reliable transportation. Motor vehicle record meeting LSS driver safety procedure. Compliance with LSS auto insurance requirements. Language Skills Read, write, analyze, and interpret documentation accurately and objectively. Present information and respond to questions from all levels of staff, clients, families, and support circles. Computer Skills/Technology Access computer systems and applications for timecards, email, and online training. Work with electronic health record(s) for documentation. Utilize varied software packages and computer systems. Physical Demands and Work Environment Bend/stoop, crouch, climb stairs, kneel frequently; access community locations. React appropriately during crises, including verbal aggression. Work in community environments and outdoors during company travel. Work environment noise is moderate.
