Registered Nurse (RN) Home Health | $15,000 Sign-On Bonus
Naples, FL | Full-Time | Assisted & Independent Living Facilities
Preferred Care Home Health is seeking a Full-Time Registered Nurse (RN) to provide skilled home health care to patients in assisted living, independent living, and other residential care communities in the Naples, FL area.

- Perform comprehensive nursing assessments and admissions for home health patients
- Develop and manage individualized plans of care alongside physicians and the care team
- Deliver skilled nursing visits per physician orders and agency policy
- Monitor patients closely and report changes in condition promptly
- Educate patients, families, and caregivers on disease management, medications, and safety
- Complete OASIS assessments and documentation accurately and on time
- Coordinate with therapists, social workers, physicians, and facility staff
- Maintain compliance with Medicare, CMS, and ACHC standards
- Participate in case conferences and interdisciplinary team meetings
- Graduate of an accredited nursing program
- Active, current Florida RN license
- Current CPR/BLS certification
- Minimum 5 years of RN experience
- Home health and OASIS documentation experience preferred
- Strong assessment, communication, and organizational skills
- Comfortable working independently while collaborating with a team
