Responsible for day-to-day nursing assessment and for the delivery and documentation of quality patient care. Current MA RN License required with minimum of 1 year of experience in ambulatory, home health, hospital or sub-acute setting preferred. A minimum of one year experience working with a frail or elderly population. Prior experience in interdisciplinary team setting a plus.
