Precision Healthcare Specialists is seeking a compassionate, organized, and patient-focused Nurse Navigator to support patients throughout their healthcare journey. The Nurse Navigator serves as a key point of contact for patients, families, providers, and interdisciplinary care teams by coordinating care, providing education, assisting with referrals, and helping remove barriers that may impact access to timely treatment.

The ideal candidate will have strong clinical knowledge, excellent communication skills, and the ability to manage multiple patients and priorities in a fast-paced healthcare environment. This position requires a high level of professionalism, attention to detail, and a commitment to providing exceptional patient-centered care.

Essential Duties and Responsibilities

• Serve as a primary point of contact for patients and their families throughout the continuum of care.
• Coordinate and facilitate timely patient care, including appointments, diagnostic testing, treatment, referrals, and follow-up services.
• Assess patients’ clinical, educational, emotional, financial, and psychosocial needs.
• Provide patients and families with clear education regarding diagnoses, treatment plans, procedures, medications, and available resources.
• Collaborate closely with physicians, advanced practice providers, medical assistants, clinical staff, specialists, and other members of the healthcare team.
• Assist patients with navigating complex healthcare systems and understanding recommended care plans.
• Identify barriers that may prevent patients from receiving timely or appropriate care and assist in developing solutions.
• Coordinate referrals to specialists, community resources, support services, financial assistance programs, and other appropriate services.
• Monitor patient progress and follow up to ensure recommended appointments, testing, treatments, and care plans are completed.
• Review patient records and clinical information to support care coordination and continuity of care.
• Communicate relevant patient concerns, changes in condition, and care needs to the appropriate provider or clinical team.
• Assist with care transitions and coordination between healthcare providers, facilities, and community resources.
• Maintain accurate, complete, and timely documentation in the electronic health record (EHR).
• Participate in multidisciplinary meetings, case reviews, care planning discussions, and quality-improvement initiatives.
• Maintain patient confidentiality and comply with all HIPAA, organizational, state, and federal requirements.
• Provide compassionate support and advocacy while promoting patient engagement and informed decision-making.
• Maintain current knowledge of clinical guidelines, treatment options, healthcare resources, and applicable standards of care.
• Perform other duties and responsibilities as assigned.

Requirements:

 

• Current and unrestricted Registered Nurse (RN) license in the State of Florida.
• Graduate of an accredited nursing program.
• Minimum of [2–3] years of clinical nursing experience , preferably in a specialty practice, outpatient setting, care coordination, case management, oncology, or another complex-care environment.
• Strong knowledge of clinical care coordination and patient education principles.
• Excellent verbal and written communication skills.
• Strong organizational, critical-thinking, and problem-solving abilities.
• Ability to manage multiple patients, responsibilities, and competing priorities effectively.
• Ability to work independently while collaborating closely with an interdisciplinary healthcare team.
• Proficiency with electronic health records (EHRs) and standard computer applications.
• Strong attention to detail and commitment to providing high-quality patient care.

Preferred Qualifications

• Bachelor of Science in Nursing (BSN).
• Previous experience as a Nurse Navigator, Care Coordinator, Case Manager, or Clinical Care Coordinator.
• Experience working with patients who have complex, chronic, or specialty healthcare needs.
• Certification in nursing navigation, case management, care coordination, or a related specialty.
• Experience in an outpatient or specialty medical practice.
• Bilingual abilities are a plus.

Physical Requirements

• Ability to remain seated or standing for extended periods.
• Ability to communicate effectively with patients, families, providers, and team members.
• Ability to use a computer, telephone, and other standard office and clinical equipment.
• Ability to move throughout the office or clinical setting as needed.
• Ability to perform the essential duties of the position with or without reasonable accommodation.

Work Environment

This position primarily works in a professional outpatient healthcare setting. The Nurse Navigator may interact with patients and healthcare team members in person, by telephone, through electronic communication, and through the electronic health record system. The position may require managing multiple priorities in a fast-paced environment while maintaining professionalism, accuracy, and patient confidentiality.
