Position Summary Under the direction of the RN Case Manager, the LPN provides nursing services, patient care activities, and accommodation services in accordance with standards, regulatory guidelines and licensure requirements. The LPN follows the plan of care while providing services, communicates with management changes, and collaborates with patient/family members, physicians or Nurse Practitioners as needed or directed. Minimal Education: Unencumbered Virginia license as a Licensed Practical Nurse Graduate of an accredited school of nursing.
