Responsibilities Provide direct care for all assigned patients, including initiation and maintenance of all communications with appropriate care team members such as pharmacists, prescribers, dietitians, home patient representatives, and community resource staff. Evaluate and educate patient/care partner to safely administer home infusion therapy and provide additional instruction as indicated. Evaluate the home environment and identify interventions required to render the care setting safe and appropriate. Develop, in conjunction with team members, the plan of care and evaluate response to therapy at each encounter. Manage schedule via company–approved platform per policy and communicate any scheduling changes promptly. Coordinate patient care via approved company platform. Participate in nurse on‑call rotation to ensure continuous nursing support services. Document all patient encounters, communications, and care activities at time of service as per policy and regulatory requirements. Maintain strong knowledge of Soleo therapies, protocols, and clinical standards. Ensure timely submission of plans of treatment (POT) and required clinical documents per policy and state guidelines. Work with appropriate departments to ensure all required patient documents (e.g., consents, POTs) are completed and returned within an acceptable timeframe. Schedule Part‑time, 30–32 hours per week, Monday to Friday with willingness to work one Saturday per month. Must be able to participate in on‑call rotation. Must be knowledgeable of IV access devices (IV lines, ports, and infusion medication administration). Must be able to drive up to 100 miles round‑trip as needed (mileage reimbursement and hourly rate provided). Must be licensed as an RN in the state of Texas. Requirements Current RN license in good standing. RN degree; bachelor’s degree in nursing (BSN) from an accredited institution preferred. Advanced certification in specialty areas of practice (e.g., CRNI, ONS, IgRN) preferred but not required. Participation in on‑call rotation, nights and weekends, to provide as‑needed support outside of regular hours. Administrative oversight: responsible for monitoring workflows, ensuring compliance with policies, and managing resources efficiently. Emergency availability: must respond promptly to emergencies, including nights, weekends, and holidays. Minimum 3 years nursing experience with background in infusion therapy preferred. Experience in ICU and/or cardiac care preferred. Proficient with Microsoft Outlook and Office Suite. Valid state‑issued driver’s license with no significant restrictions.
