Primrose Hospice Care is a locally owned, mission-driven hospice led by experienced healthcare providers who believe in restoring the personal, compassionate, patient-centered care that hospice was meant to be.

We are growing and seeking an Experienced Hospice Registered Nurse (RN) Case Manager to join our team and provide skilled, compassionate care to patients and families across the Portland metro area. ⭐ What You’ll Do

• Provide skilled nursing care to hospice patients in homes and facilities
• Complete comprehensive assessments and develop individualized Plans of Care
• Manage pain and symptoms with a holistic, patient-centered approach
• Educate patients and families about the disease process, medications, and comfort measures
• Serve as the primary case manager and coordinate care with the interdisciplinary team
• Communicate changes in condition to the attending physician and IDG
• Supervise CNAs and LPNs involved in the patient’s care
• Document timely and accurately in the EMR system
• Participate in IDG meetings, care conferences, and quality initiatives
• Rotate after-hours / on-call responsibilities as assigned

⭐ What We’re Looking For

• Current Oregon Registered Nurse (RN) license
• Minimum of 1 year of experience as a direct Hospice RN Case Manager
• Strong assessment, communication, and critical-thinking skills
• Ability to work independently and manage a caseload effectively
• Compassion for patients and families during end-of-life care
• Valid driver’s license, reliable vehicle, and auto insurance
• BSN preferred; MSN desired
