Learn more at Mental Health Case Manager Chicago | Contracted We're hiring a Mental Health Case Manager to help clients connect with the behavioral health, housing, medical, and social services they need to thrive. You'll be a trusted advocate, navigator, and coordinator across our clients' care networks. What You'll Do Assess client needs across behavioral health, medical, housing, and social services. Build individualized case-management plans aligned with treatment goals. Connect clients to community resources and confirm successful linkage. Coordinate with hospitals, providers, schools, and community organizations. Advocate for clients facing barriers to needed services. What You'll Bring Meets the qualification level required for the assigned Case Management service. Bachelor's degree in social work, psychology, sociology, human services, or related field preferred. Experience coordinating behavioral health, healthcare, or social services. Knowledge of Chicago and Cook County community resources. Nice to Have Experience with Medicaid behavioral health services. Background in discharge planning, housing navigation, or public benefits.
