Physical Therapist (PRN)

Role Physical Therapist providing age-appropriate hospice services including assessments, direct treatment, development of home exercise programs, patient/family education, adaptive equipment evaluation, and documentation according to hospice standards. Responsibilities Conduct evaluations based on physician orders to manage symptoms and maintain patient activities Document visits, patient reactions, and status changes; report to physicians and administrators Participate in Interdisciplinary Group discussions and contribute to the patient's Plan of Care Educate patients, families, and hospice team members on therapy goals and progress Prepare discharge summaries and provide therapy instructions when appropriate Qualifications & Skills Current state licensure in Physical Therapy CPR certification Ability to work effectively in diverse home environments Valid driver’s license, vehicle insurance, and access to reliable transportation Preferred: multitasking, independence, strong communication, writing, and organizational skills Key Attributes: Compassion, independence, strong communication, adaptability, and organizational skills. #J-18808-Ljbffr The VIOS Clinic
