About This Role The Community Health Nurse provides community-based professional nursing care to patients in various settings, including homes and shelters, focusing on chronic health conditions. This role is ideal for nurses who are dedicated to improving patient health outcomes through direct care and education. Highlights Pay: $112,776/year Location: Bronx, NY Department/Unit: AT HOME/HC - ADULT QUEENS Schedule: Full-Time, Day shift (9:00 A.M – 5:30 P.M) What You'll Do Provide community-based nursing care to patients in homes and shelters. Diagnose and treat responses to health problems using nursing methods. Develop, implement, and evaluate nursing regimens for patients. Complete comprehensive patient assessments and maintain care records. Record and maintain nursing care plans and progress notes. Coordinate care plans with health team members and initiate care conferences. Liaise with other health professionals to ensure continuity of care. Assign components of nursing regimens to allied nursing personnel. Provide teaching and guidance to allied nursing personnel. Participate in quality assurance and performance improvement activities. Requirements Experience: One (1) year of experience as a registered nurse required. Minimum two (2) years of full-time nursing experience preferred. At least 2 years of experience in Homecare, Long-term Care, or Hospital setting preferred. Licenses & certifications: A valid New York State license and current registration to practice as a Registered Professional Nurse in New York State.
