• Monday–Friday | 10:00 AM – 7:00 PM
• Tuesday–Saturday | 8:00 AM – 5:00 PM

We are seeking an experienced Ambulatory Care / Care Manager Registered Nurse for a 13-week travel assignment in Juneau, Alaska.

Job Responsibilities:

• Coordinate and manage patient care across the continuum of care
• Develop, implement, and monitor individualized care plans
• Collaborate with providers, patients, families, and interdisciplinary teams to ensure quality outcomes
• Provide patient education, discharge planning, and care coordination
• Maintain accurate documentation and ensure compliance with organizational standards
• Advocate for patients and assist with accessing appropriate healthcare resources

Requirements:

• Active Alaska RN License required at the time of submission
• Associate's Degree in Nursing from an accredited program
• Current, unrestricted Alaska Registered Nurse license
• Current BLS Certification
• Minimum 1 year of RN experience
• Case Management experience within the last 2 years is required
• Proof of current Flu Vaccination is required
