Responsibilities:

  • Provide direct patient care, including administration of medications, wound care, and vital sign monitoring

  • Develop and implement individualized care plans in collaboration with the interdisciplinary team

  • Conduct patient assessments and report changes to the nursing team

  • Collaborate with other healthcare professionals to ensure comprehensive care

  • Maintain accurate and detailed records of patient care and treatment

As a Licensed Practical Nurse (LPN) at Stillwater Skilled Nursing and Rehabilitation, you will have the opportunity to work with a diverse patient population and make a meaningful difference in their lives. Requirements:

  • Current LPN license in the state of Ohio

  • High school diploma or equivalent required

  • Strong communication and interpersonal skills

  • Ability to work effectively in a fast-paced environment

  • Familiarity with electronic medical records systems (EMRs) a plus
