Patient opportunities may include Bethlehem, Easton, Nazareth, Hellertown, Pen Argyl, Wind Gap, Bangor, Mount Bethel, Roseto, East Bangor, and surrounding communities. - Complete skilled nursing visits in patients' homes
- Perform comprehensive nursing assessments
- Complete Start of Care, Recertification, Resumption of Care, and Discharge visits as assigned
- Develop and update individualized plans of care
- Provide wound care, medication management, and disease education
- Monitor changes in patient condition and communicate concerns promptly
- Educate patients and caregivers
- Coordinate care with physicians, therapists, and other members of the care team
- Complete accurate and timely documentation using WellSky
- Active Pennsylvania Registered Nurse (RN) License Required
- Graduate of an accredited nursing program
- Current CPR/BLS Certification
- Valid Driver's License and reliable transportation
- Strong clinical assessment and communication skills
- Ability to work independently in patients' homes
- Home Health and OASIS Experience Preferred, but Not Required
- WellSky Experience is a Plus
