As an RN Case Manager - Home Health with UnityPoint at Home, you'll provide one-on-one care to patients with total autonomy over your role. With a flexible schedule and independence, our home care nurses are in charge of everything from patient admission to the care plan to discharge and make it possible for patients to receive care where they're most comfortable – in their homes. The RN Case Manager - Home Health uses clinical nursing knowledge, physical assessment, teaching and procedural skills to deliver high quality patient care in the patient's place of residence. The RN Case Manager - Home Health develops and oversees the home health plan of care under the direction of the physician and ensure appropriate interdisciplinary involvement to ensure the best outcome for each patient. Find a fulfilling career and make a difference with UnityPoint Health.ResponsibilitiesConduct skilled home visits to assess patient condition, provide ongoing nursing evaluation, and deliver care aligned with individualized plans of care.Complete comprehensive initial and ongoing assessments, identifying changes in condition and coordinating timely updates with physicians and the care team.Provide hands-on skilled nursing care, patient education, and caregiver training on disease management, medications, safety, equipment use, and emergency preparedness.Develop, implement, and continuously revise individualized care and discharge plans with measurable goals, ensuring patient and family involvement in all decisions.Promote patient safety and independence while proactively identifying risks for hospitalization and intervening to prevent avoidable complications.QualificationsEducation: Graduate of State Board approved program for Registered NursesLicensure/CertificationsValid license as RN in state(s) where providing care.Valid licensed driver with automobile insurance in accordance with state(s) and/or organizational requirements.Mandatory Reporter: Child & Dependent Adult Abuse.CPR: Maintain a valid Basic Life Support (BLS) Healthcare Provider Card with Re-certification.Minimum ExperienceOne year nursing experience caring for similar patient population as to be assigned.OtherUse of usual and customary equipment used to perform essential functions of the position.Apply Today!
