Physician Assistant

Signify Health is seeking a mission-driven Physician Assistant to perform comprehensive in-home health evaluations for diverse, often vulnerable, patient populations. You will conduct detailed histories, physicals, and medication reviews; identify care gaps; and document findings using Signify Health’s digital tools to support value-based care. Partnering with physicians, care coordinators, and health plan partners, you’ll help shape individualized care plans and educate patients on chronic disease management.
