Salary is $50.00-$65.00Responsibilities: As a RN Case Manager at Snowline Health, you will be responsible for providing compassionate and comprehensive nursing care to our patients and their families. Your duties will include:Empower, educate and coordinate all care details of your hospice patientsCalm, soothe and empathize with the strong emotions our patients and families are experiencingMotivate and encourage a team of healthcare professionals in providing the best care possible to each and every patientAddress pain, comfort and manage symptomsCollaborate on cases, involvement in interdisciplinary meetings, training and development and other organizational activitiesAssist with the management of clinical notes, health records, and systems to ensure appropriate and accurate dates.RequirementsBSN, Associate Nursing Degree, or a diploma from an accredited school of nursingValid California Registered Nursing LicenseMinimum one year experience in nursing (one year hospice experience preferred)Self-directed and able to work with minimal supervision, ability to organize and prioritize workload independentlyExcellent observation, problem solving, verbal and written communication skillsDemonstrated ability to efficiently use a computer, prior electronic medical record (EMR) experience preferredCurrent BLS/CPR certificationIs comfortable traveling weekly throughout the El Dorado County areaValid CA driver's license required.
