Job Description

Responsibilities include:

• Perform comprehensive annual health risk assessments in patient homes, primarily for Medicare Advantage members.
• Thoroughly review, evaluate, and document prior and current medical histories, medical conditions, and medication regimens.
• Educate members on their medical conditions and prescribed medications during each encounter.
• Detect gaps in care and suggest meaningful interventions to improve health outcomes.
• Focus exclusively on assessmentsno need to develop treatment plans or prescribe medications.

Required qualifications:

• Unrestricted Nurse Practitioner License in the state of Ohio.
• Board-certified by AANP or ANCC.
• Current, valid CPR certification (BLS).
• Master of Science in Nursing (MSN) from an accredited institution.
• Minimum of 6 months NP experience preferred.

This role offers $80$90 per completed evaluation, along with a mileage stipend and compensation for administrative duties.
