Responsibilities
- Provide direct resident care in accordance with individualized care plans and facility policies.
- Monitor resident status, perform routine assessments, document vitals, and report changes to the supervising nurse.
- Administer prescribed medications and treatments in accordance with nursing standards and physician orders.
- Assist residents with activities of daily living and mobility, promoting comfort and safety.
- Collaborate with RNs, charge nurses, and multidisciplinary team to implement and adjust care plans.
- Maintain accurate, timely documentation and participate in shift handoffs to ensure continuity of care.
- Adhere to infection control, safety, and privacy standards; respond promptly to emergencies or changes in condition.

Requirements
- Active Licensed Practical Nurse license in good standing; Vermont licensure or eligibility to practice in Vermont.
- Graduation from an approved practical nursing program (diploma or certificate).
- Prior long-term care or skilled nursing experience preferred.
- Ability to work night shift and meet the 40-hour weekly schedule.
- Strong communication, observation, and documentation skills; ability to work as part of an interdisciplinary team.
