Multi‑State Nurse Practitioner (NP) – Contracted Location: Remote – United States Job Highlights Anticipated Start Date: Summer 2026 Role Type: Contracted, 1099 Payment Terms: Net 30 Licensure: Must hold valid, unencumbered nurse practitioner licensure in AL, UT, IA, IL, IN, MI, GA, FL. Multi‑state licensure required. Schedule: Flexible, 1099; set your own hours; ideally 15–20+ hours of weekly availability. Current Demand: Monday–Friday, 9 a.m.–9 p.m. ET. Requirement: Provide availability for the next two months by the 15th of each month. Compensation: $30.00 per completed consult + $30.00/hour for non‑clinical time. Remote Requirements: Must reside in the United States, have a clean, quiet workspace, reliable internet, and proficient technical experience. We provide expert diabetes education and clinical management for Medicare patients, delivered by a comprehensive team of Registered Nurses, Registered Dietitians, and Accountability Coaches. Our model integrates high‑level clinical expertise with real‑time data from cellular‑connected glucometers and advanced CGM management to deliver life‑changing outcomes. About the Role We are seeking an experienced Nurse Practitioner (NP) passionate about remote care delivery. In this role, you will lead diabetes education and management by conducting phone visits for initial member consultations, evaluating diagnoses, medications, and lab results to assess member acuity, validate candidacy and continuation in our clinical program, and identify and prescribe CGMs (Continuous Glucose Monitors) for applicable members. Key Responsibilities Provide call‑based care to members in the Remote Patient Monitoring (RPM) program. Deliver swift, empathic care and assess members’ clinical competencies. Evaluate medical history and data to determine member eligibility for the clinical program. Serve as the primary clinical point of contact for practice‑wide escalations. Collaborate with the broader care team – including RNs, RDs, and Accountability Coaches – to reinforce clinical goals and SMART goal progression. Adhere to Perry Health policies, procedures, and standard operating procedures to ensure ongoing clinical quality and compliance. Qualifications Education: Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited institution; active certification as a Nurse Practitioner. License: Valid, unencumbered state NP license required; multi‑state licensure, with priority in AL, UT, IA. Experience: Minimum 3+ years managing patients with diabetes. Knowledge: Familiarity with CGMs and behavior change theory is a plus. Skills: Motivated learner, willing to acquire proficiency with required technology; experience or training in Motivational Interviewing is a plus. Traits: Creative thinking, attention to detail, ability to work independently, positive attitude, and thrive in a fast‑paced environment while collaborating with diverse groups. Language: Spanish‑speaking highly preferred. Compensation & Payment Terms Contractors are paid monthly, following Net 30 payment terms. Clinical Pay: $30.00 per completed consult. Consults are scheduled in 20‑minute slots and require a minimum of 15 minutes of engagement time per member for the visit to be billable.
