Somatus, a leader in chronic kidney disease care, is hiring a CHW-Patient Advocate for a Hybrid Telehealth role focused on CKD and ESKD populations. You’ll partner with RN Care Manager and physicians to coordinate care, schedule wellness exams, and connect patients with community resources.
