Somatus - Revolutionizing Kidney Care

Somatus, a leader in chronic kidney disease care, is hiring a CHW-Patient Advocate for a Hybrid Telehealth role focused on CKD and ESKD populations. You’ll partner with RN Care Manager and physicians to coordinate care, schedule wellness exams, and connect patients with community resources. You’ll engage via in-person, phone, and online outreach; manage a caseload, travel up to 50–75% for home visits, and help patients overcome language and cultural barriers while promoting self-care and #J-18808-Ljbffr Somatus - Revolutionizing Kidney Care
