Position Overview

This role involves coordinating and directing home nursing care according to physician orders, adhering to agency policies and procedures through the competent application of the nursing process. The position is responsible to the Clinical Manager and includes direct supervision and evaluation of Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.

Key Responsibilities

• Provide comprehensive home nursing care based on patient needs and physician directives.
• Supervise and evaluate the performance of Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
• Conduct home visits, ensuring adherence to established protocols and quality of care.
• Participate in periodic office meetings for team coordination and care planning.

Travel Requirements

This role requires reliable transportation and a valid driver's license as home visits are essential. The position involves travel up to 75 miles from the assigned clinic, with mileage reimbursement provided at the IRS standard rate.

Caseload and Productivity

• Average number of patients per day: 5-10
• Weekly productivity expectation: 30 points, with different point values assigned to each type of visit and activity.
• All visits must be documented within 24 hours to ensure quality care and compliance.

Requirements

• Minimum of 1 year of adult home health experience or hospice experience, depending on specialty.
• Current certification in CPR and BLS.
• Proficiency in HomeCare HomeBase (HCHB) required, with recent utilization within the last 6 months.
• For RNs, proficiency in OASIS is essential.

Working Conditions

This position is primarily day shifts, Monday through Friday, from 8:00 AM to 5:00 PM, with some on-call rotation and weekend coverage required depending on location.
