Job Details Level: Experienced Job Location: St Helens, OR 97051 Position Type: Full Time Salary Range: $23.00 - $28.00 Hourly Job Category: Health Care Position Overview The Certified Nurse Assistant provides nursing and nursing‑related services to residents consistent with each resident’s comprehensive resident assessment and plan of care. All resident care is provided in a manner that meets the resident’s physical, mental and psychosocial needs and enables the individual to attain or maintain the highest practicable level of functioning. The Certified Nurse Assistant maintains a “homelike” environment for the residents, protects and promotes resident rights and assists the person to maintain independence and control to the greatest extent possible. Reporting This position is responsible to the Director of Staff Development and/or Director of Nursing. FLSA Status Non-Exempt Essential Job Functions Provide direct care to residents to improve or maintain the resident’s abilities according to the resident’s plan of care. Activities of Daily Living (ADLs) Assist with bathing, grooming, oral care, dressing, mobility, transferring, and feeding of assigned residents. Include set‑up assistance, verbal prompts, physical support, or extensive assistance as required. Bathing – Assist transport of resident to tub or shower, adjust water temperature, wash and rinse, dry body thoroughly with clean towels, maintain privacy at all times. Dressing – Locate, select, and obtain clothing; assist with snaps, zippers, buttons; recognize clothing in need of repair and communicate needs to resident and Charge Nurse. Restorative Care – Assist resident to maintain functioning at highest independent level, prevent decreases in function, use ambulation, range‑of‑motion exercises, assistive devices, proper body alignment. Elimination/Toileting – Promptly assist residents to bathroom according to schedule or bring clean bedpan/urinal; open, remove clothing, clean residents if necessary; measure and record output; provide catheter care per procedure and infection control policies. Mobility – Assist residents or provide range‑of‑motion exercises, weight‑bearing assistance, or physical support for ambulating; push wheel chair or geri‑chair; maintain safe environment, remove obstacles, inspect equipment, report defects. Transfer – Position residents, use correct transfer technique and equipment, obtain assistance when needed before starting transfer. Eating – Provide set‑up assistance for meal tray, open, pour, unwrap, cut food if residents cannot help self; feed individuals requiring total assistance; encourage chewing, eating, swallowing; assist or clean residents after meals; serve nourishment and snacks as ordered; document and report intake. Provide care that protects and promotes resident rights, dignity, self‑determination, and active participation. Offer respect for resident choices; refer to residents by proper names; avoid pet names; handle residents’ property with respect. Observe and report patterns of decreased social interaction, anger or sadness, and refusals of treatment to Unit Charge Nurse. Assist with planning, implementation, and evaluation of plans of care; report information on conditions and responses to interventions, suggest alternative approaches. Assist with preparation for state inspections; attend survey training and interact with state surveyors per supervision. Follow all facility policies and procedures including infection control; use personal protective equipment when indicated. Record accurate, legible information about resident care in appropriate sections of resident’s records. Communicate with Charge Nurse about assigned residents regularly throughout shift. Demonstrate competence in proper body mechanics; safely and correctly use equipment such as back belts, gait belts, and mechanical lifting equipment for residents ranging from less than80pounds to over300pounds. Monitor physical environment for safety or fire hazards; correct or report problems immediately to Charge Nurse. Report injury to resident or self to Charge Nurse immediately. Attend 12hours of in‑service education provided annually; apply information to job tasks. Participate in fire and disaster drills; carry out assigned duties to assure resident safety in emergencies. Job Functions Demonstrate knowledge of, and respect for, the rights, dignity and individuality of each resident in all interactions. Appreciate importance of maintaining confidentiality of resident and facility information. Demonstrate honesty and integrity at all times in the care and use of resident and facility property. Understand and follow written and verbal directions; communicate effectively with staff members and residents. Interact effectively with individuals who are cognitively and/or physically impaired. Knowledge of emergency and disaster procedures; locate nearest exit; understand and respond to written or oral instruction. Show sufficient mobility and strength to move freely through the building; assist, transfer or otherwise move residents of facility out of danger in emergencies. Respond to needs of residents while complying with facility policies on attendance, punctuality and dress code; arrive on time. Comply with workplace safety policies including infection control; standard precautions for bloodborne pathogens; use personal protective equipment; handle hazardous materials. Prioritize tasks, complete duties within allotted time, and respond productively to change. Carry out essential functions without posing risk of harm to self or others. Demonstrate competence in performing all services required of a nursing assistant, including care for individuals with cognitive impairments. Recognize changes in resident’s condition via physical symptoms, verbal expression or observation of mental/psychosocial status. Other duties as assigned. Physical Capacities See and hear or use prosthetics to function adequately. Meet general health requirements, including annual TB screening and physical examination. Lift a minimum of 25pounds. Lift and over 100pounds with assistance per resident’s abilities. Move intermittently during working hours. Stand and walk for extended periods. Bend, squat, push, stoop, and pull as needed, especially during resident care activities. Coping with mental and emotional stress of the position. Environmental Conditions Inside work, normal temperatures, occasional noise, fumes/odors, chemical exposure, and potential exposure to bloodborne pathogens. The job requires familiarity with emergency health or safety response, weekend and holiday duty, unusual or impaired behaviors, family reactions, death and dying, oversight by state surveyors, ombudsmen, and federal officials, and variable involvement of medical staff. Qualifications / Requirements Education: Sufficient education to demonstrate functional literacy. Current CNA certification issued by the local Department of Health Services or proof of completion of an approved OBRA certification program from another state. No findings in the State Nurse Aide Registry concerning abuse, neglect, or misuse of resident property. Prior experience giving personal care to impaired individuals in a healthcare setting (preferred). Language Skills: Able to read, analyze, and interpret common scientific and technical information; communicate verbally in English. Mathematical Skills: Add, subtract, multiply, divide in all units of measure; use whole numbers, common fractions, decimals, weight, and volume.
