Implement therapeutic exercises, range of motion activities, gait training, mobility work, and transfer coaching. Support activities of daily living and functional mobility goals as part of the rehabilitation plan. Monitor patient responses to treatment, document progress, and communicate changes to the supervising PT. Collaborate with the rehabilitation team and nursing staff to ensure coordinated, patient-centered care. Maintain accurate, timely documentation and comply with safety, privacy, and clinical regulations. Requirements Active Physical Therapy Assistant license in New Hampshire or eligibility to obtain licensure. Ability to work in a LTAC/long-term care/assisted living setting. Strong communication and teamwork skills; good documentation habits. Availability for the 8:00 a.m. – 4:30 p.m. shift (5 days per week) and willingness to commit to a 13-week assignment with 35 guaranteed hours. Travel-capable and prepared for a temporary assignment in a different location.
