Location: Fort Dodge, Iowa
Job Type: Permanent | Full-Time
Pay Rate: $37.82–$56.74/hour
Sign-On Bonus: $20,000
Relocation Assistance: Potential assistance up to $10,000
Schedule
- Monday–Friday
- 8:00 AM–4:30 PM
- 8-hour shifts
- Weekend rotation required
- No on-call requirements
Key Responsibilities
- Visit patients in their homes and assess their occupational therapy needs.
- Evaluate patients' functional abilities using appropriate tests.
- Develop and implement individualized treatment plans based on physician orders and patient evaluations.
- Recommend environmental modifications to support patient independence.
Requirements
- Graduate of an ACOTE-accredited Occupational Therapy program.
- Active Iowa OT license or eligibility under applicable state requirements.
- Valid driver's license and automobile insurance.
- Current BLS certification.
- At least 1 year of experience caring for a similar patient population.
- Mandatory Reporter certification for Child and Dependent Adult Abuse.
- NBCOT certification or eligibility to take the examination is highly preferred.
