Job Summary:

Oasis at the Keys Nursing and Rehab is seeking a dedicated and compassionate Physical Therapist to join our team in Tavernier, Florida. Responsibilities:

• Assess referred patients that require rehabilitation services and develop personalized treatment plans

• Provide direct patient care, including evaluations, treatments, and education

• Establish and administer treatment programs with specific goals determined according to patients' capacity and tolerance

• Document and record patients' conditions, progress, and treatment plans

• Collaborate with other disciplines to provide patient-centered care

• Meet all regulatory requirements, including evaluations, weekly progress notes, recertification, and discharge documentations

• Perform all other duties assigned by the Director of Rehabilitation

Requirements/Qualifications:

• Graduation from an accredited Physical Therapy college or university

• Valid professional license and registration for the state of practice

• Experience with long-term care and/or geriatric care is preferred, but not limited to

• Experience with PPS scheduling is preferred, but not limited to
