Importance of This Role

As a Home Health Physical Therapist, you play an essential role in the care team, aiding patients in their recovery, restoring mobility, and helping them maintain independence in their preferred environment: their home.

 

This position offers a high degree of autonomy and is centered around building relationships, allowing you to witness the effects of your work on a daily basis.

 

Your Responsibilities

Provide Patient-Centered Care

• Assess and evaluate patients’ functional abilities, mobility, strength, and rehabilitation potential.
• Collaborate with physicians and the care team to create and implement personalized care plans.
• Administer hands-on therapy to enhance mobility, strength, balance, and independence.
• Educate patients and their caregivers about treatment plans, exercises, and safety measures.

Enhance Outcomes & Recovery

• Track patient progress and modify treatment plans as necessary.
• Facilitate safe transitions of care and assist in discharge planning.
• Identify additional patient needs and work with interdisciplinary teams.

Collaborate & Communicate

• Work closely with nurses, therapists, and physicians to ensure cohesive care.
• Accurately document patient care and progress in the electronic medical record (EMR).
• Engage in case conferences and team meetings.

Lead & Support

• Oversee Physical Therapist Assistants (PTAs) in line with state regulations.
• Offer guidance and training to caregivers and support staff.

 

Your Qualifications

• Graduation from an accredited Physical Therapy program (DPT or Master’s).
• Active PT license in the state where you practice.
• 1–2 years of experience preferred, particularly in home health.
• Strong clinical assessment and critical thinking abilities.
• Excellent communication skills and patient education capabilities.
• Current CPR certification.
• Valid driver’s license, dependable transportation, and auto insurance.
