Patient care may involve evaluations, administering prescribed treatments and therapies, care planning, communication with other members of the patients multi-disciplinary team, emergency management, coordination of care, health promotion and teaching and training family members.Key ResponsibilitiesProvide Speech Therapy services as prescribed by the patients physicianMaintain a full time case load of at least 44 treatment visits plus 3 treatment visits weekly (47 visits weekly).Provide a written evaluation defining Speech Therapy needs, goals and expected outcomes in conjunction with referring physicians plan of treatmentFollow policies and procedures and adheres to all applicable patient care standards, licensure regulations and state lawsDevelop a written plan of care for Speech Therapy with reasonable goals and outcomesMaintain and adjust the plan of care as needed for assigned patients according to identified Speech Therapy goals and outcomesMaintain patient and company confidentialityMaintain effective communication between staff, health care team members and familyRefer patients when appropriate to additional educational or medical servicesParticipate in coordination of services (including discharge planning), with the health care team, physicians, parents/guardians, nurses and othersBenefitsBenefits are offered to full time employees who work 30 hours or more a week.
