This role partners with patients, families, caregivers, and the interdisciplinary team to support comfort, symptom management, and quality of life throughout the hospice experience.Essential ResponsibilitiesProvide direct patient care in accordance with physician orders and established care plansImplement current nursing practices based on comprehensive patient assessmentsAccurately and promptly document patient visits, interventions, and outcomesEducate patients and families regarding diagnoses, treatment plans, medications, and disease progressionCollaborate with the interdisciplinary hospice team, including Hospice Aides, Social Workers, Physical Therapists, Occupational Therapists, Dietitians, and Clinical SupervisorsMonitor patient conditions and communicate changes appropriatelyProvide pain and symptom management while evaluating patient responses to treatments and medicationsSupport patients and families with compassionate, patient-centered care throughout the hospice journeyQualificationsGraduate of an accredited Practical Nursing or Vocational Nursing programCurrent LPN license in the state of OhioMinimum one year of medical-surgical nursing experience requiredPrevious hospice or home health experience preferredCurrent CPR certificationValid driver's license and reliable, insured transportation meeting organizational requirementsWhy Capital City Hospice?At Capital City Hospice, you'll find more than just a job.
