Work Environment

You will be part of a collaborative team within our outpatient clinic, which includes physicians, fellow nurse practitioners, physician assistants, medical assistants, a dedicated medical scribe, registered nurses, and care managers. This team-oriented environment allows our healthcare providers to concentrate on primary care, care coordination, and addressing population health needs, all while partnering with a network of specialists and hospitals to streamline specialty care access.

Key Responsibilities

• Deliver exceptional primary care by conducting thorough office visits for both acute and routine health issues.
• Administer annual wellness visits and comprehensive health risk assessments, which require accurate documentation and a holistic view of patient health.
• Coordinate care with other healthcare providers, specialists, and testing facilities to ensure seamless patient experiences.
• Lead population health initiatives alongside the care team, ensuring all eligible patients receive appropriate screenings, such as biennial breast cancer screenings.
• Assist with phone triage, patient outreach, and education on health conditions and treatment plans.
• Participate in promotional activities to enhance the organization’s visibility in the community.
• Conduct home visits as needed to ensure comprehensive patient care.
• Report directly to the Medical Director, collaborating closely with operational leadership to elevate the standard of care.

Patient Population / Client Focus

Your primary focus will be on delivering personalized care to older adults enrolled in Medicare, using an approach that emphasizes keeping these individuals healthy and engaged in their lives. Our innovative care model is community-centered, targeting quality of care over the volume of services delivered, ensuring that every patient feels supported and connected to their health care plan.

Qualifications

• A graduate degree in nursing, either a Master of Science in Nursing (MSN), Doctor of Nursing Practice (DNP), or completion of an NP Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC is mandatory.
• An active, non-probationary state Nurse Practitioner license and a current DEA license are required.
• Experience in primary care settings, particularly in internal medicine, geriatrics, or family medicine, is essential.
• Familiarity with Hierarchical Condition Categories (HCC) documentation, ICD-10 coding, and health risk assessments is highly desirable.

Compensation and Benefits

We offer a competitive salary range of USD 180,000 to 220,000 per year, adjusted based on your experience and expertise. Additional benefits include:
