• Assess patients' physical abilities and develop personalized treatment plans in the home setting
• Provide therapy to improve mobility, strength, and functional independence
• Collaborate with patients, families, and caregivers to support rehabilitation goals
• Educate patients and families on exercise programs and safety measures
• Monitor and document patient progress and adjust treatment plans as needed
• Implement interventions tailored to the home environment and lifestyle
• Coordinate care with healthcare teams and community resources for comprehensive support

Qualifications:

• Doctorate in Physical Therapy
• CA Physical Therapist PT license
• Current BLS/CPR Certification
• Clean Driving Record

Pay: $2,292.63 - $2,363.54 / week

Location: Rancho Cordova, CA

Assignment Length: 13

Shift: Day - up to 40.00 hours per week

Setting: Home Health

Workplace: On-site
