Physical Therapist - Travel Contract

Minimum Requirements Doctoral Degree in Physical Therapy (DPT) from an accredited program. This is required by the hiring manager. Active Oregon Physical Therapist license required prior to start of assignment. Current BLS (Basic Life Support) certification from AHA or ARC may be required. Home health experience preferred. Candidates with HomeCare Homebase EMR experience are preferred. Experience conducting PT visits including SOC, PT evaluations, recertifications, ROC, and supervisory visits preferred. Ability to travel within Jackson County for patient visits. Mileage reimbursed between visits from first patient to last patient.

Assignment Details Facility Type: Home Health. Shift Name: Days. Shift Details: Monday–Friday, 0800–1600 (5x8 schedule noted as 0800–1630 in client notes). Weekly Hours: 40 hour guarantee. Productivity Expectation: 30 points (client-defined productivity metric).
