Home Infusion Nurse - Per Diem - Buffalo

Infusion Nurse II Licensed registered nurse who coordinates and provides intermediate direct patient care to patients in the home or an alternate infusion suite, ensuring patient safety, continuity, and compliance with a physician’s plan of care. Responsibilities Initiate, develop, and implement intermediate nursing plan of care treatments, evaluating patient progress toward goals. Organize and participate in the provision of direct patient care, perform treatments, administer medications, and educate patients and families/caregivers. Modify plan of treatment in response to changing patient status or physician orders to achieve established or revised patient care goals. Assess patient needs and physical status at each skilled visit through health data access and patient interview. Re‑evaluate patient needs through physical reassessment, response to therapy, and supplemental physician orders. Obtain and clarify physician orders for plan of treatment revisions, inform physician promptly of significant changes in patient’s condition, and provide a written summary to physician within supplemental order. Provide training and mentorship to Infusion Nurse I’s as needed to ensure patient safety and compliance. Teach patients and families to achieve independence with prescribed therapy and care needs through active participation per plan of treatment goals. Communicate effectively with the clinical team, medical providers, patients, and families/caregivers to facilitate continuity of care. Prepare clinical documentation in real‑time during visits and submit to the nursing department to comply with established timelines for billing optimization. Complete all documentation legibly and apply approved abbreviations and documentation error‑correction practices per Option Care’s policy. Coordinate discharge planning and prepare discharge summaries with patient instructions, and thoroughly report patient care needs, progress, and goals when transferring care. Demonstrate compliance with agency operations, Option Care’s policies and procedures, professional standards, local, state, federal regulations/guidelines, and accreditation standards. Maximize work efficiency through the use of computers and other automation technologies to validate plan of treatment orders, communicate patient care provided, and follow assigned visit schedule. Respond promptly and appropriately to patient requests, initiate emergency procedures as necessary, and accept accountability for own practice through ethical and professional conduct. Follow established programs and practice within policies and procedures reflective of Option Care’s mission, values, and objectives. Observe legal and ethical guidelines for safeguarding the confidentiality of patient and proprietary Option Care information, including adherence to HIPAA regulations. Speak knowledgeably about Option Care’s scope of services and effectively instruct patients about related financial obligations for care and service charges. Attend and complete required training modules, in‑services, and continuing education to maintain competency and professional licensure. Provide oversight and coordination of paraprofessionals in the home setting and may act as the supervisor in the absence of the Supervisor or Nurse Manager. Participate in multidisciplinary team conferences and provide precepting, training, and mentoring to other nurses for orientation and onboarding as assigned. Participate in nursing department on‑call responsibilities. Perform other related duties as directed by a supervisor. If applicable: insert PICC/Midlines per physician order, troubleshoot/declot obstructed central line catheters, monitor PICC/Midline, and educate on prevention of line infection and routine care. Qualifications Active and unrestricted RN license in the state of practice. Minimum of 1 year of nursing experience. Current CPR certification required. Demonstrated competency in patient care standards and infusion skill sets applicable to agency programs. Advanced certification and training as applicable. Valid driver’s license in the state of practice with access to a vehicle for business travel and proof of liability insurance. Basic knowledge of computer operating systems and software applications. Physical ability to lift up to 50 pounds, with a maximum lifting of 75 pounds; ability to exert up to 20 pounds of force occasionally and up to 10 pounds frequently. Ability to walk, stand, crouch, kneel, turn, pivot, balance, stoop, reach overhead, grasp, push, pull, lift, and carry. Fine‑motor and visual acuity skills, including near and far vision, color and peripheral vision, depth perception, and focus adjustment. Willingness to travel up to 100% of the time for business purposes. Physical Demands Ability to lift up to 50 pounds with a maximum lifting of 75 pounds. Exerting up to 20 pounds of force occasionally, or up to 10 pounds of force frequently. Physical demands may involve walking, standing, crouching, kneeling, turning, pivoting, balancing, stooping, reaching overhead, grasping, pushing, pulling, lifting and carrying. Fine‑motor skills and visual acuity required as described above. Potential exposure to blood, body fluids and other potentially infectious materials, hazardous substances, needles, sharps, fumes, chemicals, and unpredictable home environments. Safety requirements include closed‑toe shoes, facemask, goggles, gown and/or gloves and a functioning communication device for routine and emergency situations. Preferred Qualifications & Interests Bachelor’s degree in Nursing preferred. 1–2 years of previous infusion nursing experience preferred.
