This position works closely with leadership and a broad spectrum of healthcare providers and administrative personnel to ensure optimal patient care in the satellite clinical setting. This position is housed off-site in an affiliated organizational setting with the potential to flex on-site as needed to meet patient care needs. This position works in close liaison with the Health Care Team to ensure that patient care is coordinated, comprehensive, and that care transitions are seamless. This position practices at the of their scope and actively develops, coordinates, oversees and administers care to patients in accordance within the framework of team-based patient-centered model of care. **Job Description****Minimum Qualifications:**2. Registered Nurse (RN) license.**Preferred Qualifications:****Duties and Responsibilities:** The duties and responsibilities listed below are intended to describe the general nature of work and are not intended to be an all-inclusive list. Acts as a resource for all staff in regards to education, planning, relocating and development of evidence-based, best practice clinical programs pertaining to the specialty or primary care patient population. 4. Mentors and actively participates in overseeing all clinical support staffing in the ambulatory clinical setting; delegates responsibilities appropriately and within scope. 5. In conjunction with satellite clinical and administrative leadership, participates in coordinating any transition of patient care from the satellite setting to another in collaboration with and maintaining open communication with a wide range of interdisciplinary team members with the goal of achieving an exceptional patient experience.7. Promotes shared decision making with patients in regards to healthcare; educates the patient and /or family regarding the patient's clinical condition, treatment, and their role in their healthcare decisions. 10. Demonstrates compliance with regulatory requirements i.e. MIPS, fall assessment.**Physical Requirements:**1. Location is off-site in an affiliated satellite location.2.
