Job Summary:
Physical Therapist Outpatient position in Sacramento, California. This role involves providing outpatient physical therapy services within a skilled nursing facility and inpatient acute care hospital settings. The position requires working independently to perform transfers and deliver high-quality patient care, predominantly treating patients with post-operative, neurological, and chronic medical conditions.

Job Details:

• Schedule: 8-hour days with two weekend shifts per month, starting at 08:00 AM.
• Required Qualifications: CA Physical Therapist license, American Heart Association Basic Life Support (BLS) certification, minimum of 2 years of experience, and a valid license to submit.
• Preferred Qualifications: Experience working in outpatient settings, including skilled nursing or acute care environments.
• Float: Float rotation may be required.
• Additional Information: Candidates should be able to work independently, demonstrate excellent communication skills, and have prior experience with Cerner charting system. The role involves providing outpatient therapy services focusing on total joint replacements, stroke recovery, deconditioning, and management of chronic conditions such as COPD and geriatric care.
