Assess patient condition, monitor vital signs, administer prescribed medications, and implement plan of care under supervision. Perform wound care procedures, dressing changes, and routine clinical tasks in accordance with clinical policies. Maintain accurate and timely charting in the electronic medical record; communicate updates to the RN supervisor and care team. Educate patients and families on care plans, medication administration, safety, and self-management strategies. Coordinate with physicians, therapists, and other providers to ensure continuity of care and adherence to care plans.
