In this role, you will evaluate, diagnose, and manage patients in a long-term care setting, providing proactive, comprehensive primary care to a vulnerable geriatric population.

What You'll Be Doing

  • Conduct comprehensive health assessments
  • Diagnose and manage acute and chronic conditions

What We're Looking For

  • Active NP (FNP or AGNP) or PA license
  • Experience in geriatrics, long-term care, or primary care preferred
