Participate in team Huddles, work with the team to ensure appropriate pre-visit preparation including culturally and linguistically appropriate services2. Complete intake interview for new and returning patients according to established procedure including counseling patients/families on their role, and RCH's role, in a medical home (Patient Centered Medical Home Brochure insert)3. Prepare patients for examinations consistent with the purpose of the appointment4. Record all pertinent elements of the patient encounter into EMR by providing real time charting for provider5. Responsible for knowing and following clinical policy and procedure, including documenting all patient interactions in Epic in a manner consistent with professional standards6. Perform related duties necessary to ensure quality patient care as requested by supervisor, including addressing barriers when the patient has not met treatment goals7. Assist providers with exams of patients or any special procedures8. Process phone calls or MyChart communications from patients, routing as appropriate, and responding in a timely manner9. "Ready" refill requests per Epic procedure10. Administer treatments including, nebulizer, oral medications, pediatric and adult vaccines at the direction of the RN or provider11. Perform screening tests including vision, audiometry, EKG, pulse OX, and peak flow meter at the direction of the provider12. Clean and stock rooms, disinfect exam room tables, treatment and lab surfaces daily13. Schedule qualified patients for appointments based on established protocol, identified need, and availability of appointments; follow up with patients/families who have not kept important appointments14. Perform supervised laboratory procedures as approved by the Laboratory Manager15. Prepare and sterilize clinical instruments. Review expiration dates for sterilized equipment and re-sterilize equipment as needed to ensure a readily available supply of sterile instruments16. Assist with Medication Assistance Programs17.
