Care Coordinator

Care Coordinator Full‑time, non‑exempt position. Rotating shift: 7:00 PM–7:30 AM. Location: Crisis—Franklin County Crisis Care Center, Columbus, OH. Hourly wage: $19.00–$22.00. Summary of Job The Care Coordinator facilitates the quality and improvement of a person’s experience of care, which may include admission, discharge, outreach, engagement, recovery, transition and case management. Essential Duties and Responsibilities Guest Care & Advocacy: Provides coordination of care services and resources internally and throughout the community; acts as an advocate for individuals’ needs and rights; assists individuals in daily living activities and establishing linkages that provide education, support and independence in the community. Compliance & Risk Management: Documents relevant information and services to coordinate care and communicate with team members; keeps abreast of federal and state regulations and all Recovery Innovations policies; complies with Recovery Innovations standards in the Code of Conduct and Conflict of Interest Policies. Cross‑Functional Collaboration: Coordinates or facilitates multidisciplinary team meetings to ensure timely services; partners with other provider agencies to help participants achieve wellness. Additional Duties: Performs other related duties as assigned. Qualifications Bachelor’s Degree in a behavioral health field and a minimum of one (1) year of behavioral health experience; or Bachelor’s Degree in any field and a minimum of two (2) years of behavioral health experience; or High School Diploma or equivalent and a minimum of four (4) years of behavioral health experience. Experience Ability to provide courteous assistance in a concise, informative manner, keeping a positive tone and appropriate non‑verbal communication. Works cooperatively in a team environment. Skills Excellent computer skills, including Microsoft Office Suite. Effective oral and written communication. Excellent interpersonal skills. Excellent customer service skills. Demonstrated organizational skills and high degree of accuracy. Driving Requirements If driving is required, must meet all Recovery Innovations driving policy requirements. If not qualified to drive, a Non‑Driver Attestation form must be signed. Miscellaneous Requirements Travel required based on business need. Applicants must be at least 18 years old unless state regulations set a different minimum. Maintain confidentiality per HIPAA guidelines. State‑Specific Requirements Arizona: Must be 21 years or older; provides coaching and mentoring for care coordinator staff; ensures all team members receive orientation and training; provides quality assurance and random audits of documentation. Louisiana: Must be 24 years or older. Washington: Additional specified state registration required for Registered Agency Affiliated Counselor (AAC).
