Senior Community Health Worker - Medical Home

The Senior Community Health Worker (CHW) is a trusted member of their community who acts as a liaison between community and the health system. The Senior CHW acts collaboratively with other members of the care team while demonstrating responsive communication with diverse populations. The Senior CHW has experience providing culturally appropriate education and can support community members and patients as they navigate complex social and health systems. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request. See policy for additional information. * Within 6 months (180 days) of hire: Complete the Community Health Worker Training and be certified by the state of Oregon.

The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors.
