Hartford Healthcare seeks a compassionate Hospice Admission Nurse to join our Healthcare & Medical Services team. In this field-based role, you will complete timely admission assessments, initiate individualized hospice care plans, and provide expert pain and symptom management for patients at end of life. You will educate and support patients and families, collaborate closely with our interdisciplinary team, and document care in our EMR.
