Roles & Responsibilities

• Implement individualized treatment plans developed by supervising Physical Therapists
• Deliver evidence-based interventions to improve mobility, strength, and functional independence
• Track patient progress and communicate changes to the supervising therapist
• Educate patients and families on exercises, mobility techniques, and safety
• Collaborate with physicians, nurses, and the rehab team for top-notch care
• Maintain accurate documentation in line with facility and regulatory standards

Qualifications

• Active Physical Therapist Assistant (PTA) license in Illinois
• Associate degree from an accredited PTA program
• Current CPR/BLS certification
• SNF experience preferred but not required
• Strong clinical skills and a passion for patient care

Education

• Associate Degree from an accredited Physical Therapist Assistant program

• Physical therapy and rehabilitation techniques
• Patient care and interpersonal communication
• Clinical assessment and documentation
