Physical Therapist

Role Licensed Physical Therapist providing personalized, in-home and senior living care to older adults, utilizing the Geriatric House Calls™ model. Responsibilities Deliver one-on-one physical therapy in private residences and senior communities. Perform evaluations, develop individualized treatment plans, and administer evidence-based therapies to improve mobility, independence, and quality of life. Document care accurately in EMR systems following regulatory standards. Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and fitness specialists. Educate patients, caregivers, and staff to promote safety and aging in place. Integrate telehealth and virtual services to enhance care flexibility and access. Qualifications & Skills Valid Physical Therapy license or eligibility in the practice state. Degree from an accredited Physical Therapy program. Proficiency in computer use and EMR documentation. Current CPR certification. Physical Demands Ability to lift up to 50 lbs, assist with patient transfers, perform repositioning, stand and walk for extended periods, and drive to patient locations. Use of PPE when necessary.
