Key Responsibilities

• Plan and conduct segments of treatment programs designed to restore and improve physical, social, and mental functions in alignment with department objectives
• Serve as co-leader in an interdisciplinary approach to patient care and patient groups
• Utilize HCHB and OASIS documentation platforms for patient records and clinical documentation
• Utilize EPIC for electronic health record management
• Provide home health physical therapy services to patients across the assigned service area, with mileage reimbursed at the government rate

Required Qualifications

• Doctor of Physical Therapy (DPT) degree required if license was obtained on or after January 2016, or foreign equivalent
• Active Physical Therapy licensure in the state of practice: Required
• AHA BLS or CPR certification (AHA-BLS or AHA-CPR) from an approved vendor: Required
• Valid Driver's License required; must be at least 21 years of age or older
• Minimum 1 year of home health experience required
• Proficiency in HCHB, OASIS, and EPIC required

Preferred Experience / Skills

• Experience working in an interdisciplinary or team-based care model
• Strong organizational and time management skills suited for an independent, field-based role
