RESPONSIBILITIES: Oversee the clinical care and daily operations of the Hospice unit Supervise nursing staff, nursing assistants, and unit support staff while providing leadership and guidance Monitor resident conditions, review assessments, and ensure care plans are updated and implemented appropriately Coordinate communication with physicians, nurse practitioners, physician assistants, residents, and families Review laboratory results, provider orders, incident reports, and changes in resident condition Participate in care plan meetings, QAPI activities, family meetings, and interdisciplinary team discussions Assist with admissions, discharges, transfers, and coordination of outside appointments Ensure unit supplies, documentation, and resident records are accurate and current Support staff education, orientation, infection prevention efforts, and quality improvement initiatives Address resident, family, and staff concern while promoting a positive care environment Perform additional duties as assigned REQUIREMENTS: High School Diploma or equivalent required; Associate's Degree preferred Valid NYS Licensed Practical Nurse (LPN) License required Minimum 1 year of LPN experience in a long-term care setting required 3-5 years of long-term care experience with supervisory experience preferred Strong knowledge of resident care planning, clinical documentation, and long-term care regulations Leadership experience with the ability to supervise, coach, and mentor staff Excellent communication, organizational, and problem-solving skills Ability to coordinate care across multiple disciplines and manage competing priorities Commitment to providing compassionate, resident-centered care BENEFITS: Paid Time Off (PTO) for Full-Time and Part-Time Weekly or Same-Day Pay Options Health, Dental, and Vision Insurance Life Insurance Employee Referral Bonus Program Retirement Saving Options
