Job Title: Physical Therapist (PT)
Location: Santa Clara County, CA
Setting: Home Health
Term: Temp to Hire
Shift: 24-28 visits weekly
Compensation: $70-75/hour

Astrya Global, a San Diego-based medical staffing agency, is looking for Physical Therapist (PT) for a home health role that would provide physical therapy services to senior patients (65+) in Santa Clara County. The ideal candidate will have previous Home Health experience. Enjoy competitive pay, plus a company-provided laptop to support documentation.

• Weekly Pay: $70.00 - $75.00/hr

Qualifications of a Physical Therapist (PT):
• Current CA, PT License
• Minimum 2 years' experience in home health (preferred)
• Driver's License
• BLS Certification
• Auto Insurance

Duties/Responsibilities of a Physical Therapist (PT):
• Perform comprehensive in-home physical therapy evaluations for elderly patients
• Develop and implement individualized treatment plans focused on strength, balance, mobility, and functional independence
• Provide therapy for conditions including post-surgical recovery, stroke, arthritis, neurological disorders, and chronic pain
• Conduct fall risk assessments and implement fall-prevention strategies
• Instruct patients and caregivers on safe mobility, transfers, and use of assistive devices (walkers, canes, wheelchairs)
• Document visits, progress notes, and discharge summaries in compliance with Medicare and home health regulations
